| Investigation result | Power failures affected both the Resins and Olefins sites. The Olefins power loss resulted in off-specification product having to be sent to flare. |
| Response | Power restored to both sites enabling plants to restart and establish normal operations. Review of plant operations and implementation of flare management procedures. The flare operation is managed to minimise noise impacts while avoiding smoking. Follow-up discussions with caller. Posting made to Qenos Altona Facebook page to advise community. |
